FAQs
The shortest answer: dementia care facility is built around the mind, and Parkinson's care facility is built around movement. Everything else follows from that.
Parkinson's disease is a movement disorder. It a!ects gait, balance, tremor, rigidity, handwriting, facial expression, speech volume and swallowing. In the earlier years, cognition is usually intact, which means your parent knows exactly what is happening to their body and is fully capable of directing their own care. The frustration is that of a sharp mind inside a body that has stopped obeying it. Care that does not respect that autonomy is experienced as an insult, however kindly it is delivered.
Dementia works the other way. The body may be capable for years while memory, judgement, language and orientation decline. Care shifts steadily from the resident to the family, and the daily work is about safety, orientation and reading behaviour as communication.
The one thing that separates competent Parkinson's care from the rest
Medication timing. Parkinson's medication has to be given at the resident's own
prescribed times, not at the facility's convenient drug round. A dose given an
hour late can mean an afternoon in which your parents cannot stand up unaided,
and repeated late doses compound into avoidable decline. Hospitals in the
UK and US now treat Parkinson's drugs as time-critical for exactly this reason.
Most senior living facilities in India run medications on fixed rounds because
that is operationally simpler. Ask any community you are considering how they
handle a resident whose doses fall at 7:15, 11:15, 3:15 and 7:15, and whether
that schedule survives a sta! shift change. The answer tells you a great deal.
Where the two overlap
A large proportion of people with Parkinson's develop cognitive changes in
the later years, and Parkinson's disease dementia and Lewy body dementia
sit close together clinically. So a community o!ering Parkinson's care
without any dementia capability will eventually have to hand your parent
to someone else, at precisely the point when a move is most disruptive.
There is also one safety issue worth knowing about, because it is where the two disciplines genuinely diverge. People with Parkinson's and with Lewy body dementia are highly sensitive to certain older antipsychotic medications, which can worsen rigidity and movement sharply. A team used to managing agitation in general dementia, but not trained in Parkinson's, can reach for the wrong tool in good faith. Any decision here belongs to the treating neurologist or physician, not to the facility, and the community's job is to know enough to raise the question.
